Part Time Community Care Officer
The team is seeking to employ a Community Care Officer (CCO) to assist them to support adults with Asperger Syndrome.
You will be expected to work in a person centred manner to carry out community care assessments and develop support plans with the aim of enabling individual's to maintain their independence.
The role will require you to commission services where appropriate and subject to meeting the department’s eligibility criteria. Assessment will mainly take place in the individual’s home and require you to work flexibly. You will also work closely with health colleagues and in particular with carers, offering them advice and support.
Successful candidates will be provided with a comprehensive induction programme, training, and other developmental opportunities to enable them to complete their role and raise their competency level.
You will have access to regular support and supervision from a senior practitioner and a team manager as well as have access to the department’s policies and procedures.
