We are looking for an enthusiastic and highly motivated Social Worker who wants to make a real difference to the lives of vulnerable adults who are recovering from a period of ill health and/or coming to terms with long term disability.

We are seeking to appoint a qualified and registered Social Worker (Newly Qualified or Experienced) to join an established and supportive team which provides assessments to both younger and older adults with complex needs and recovering from a period ill health. The post holder will be required to work alongside a variety of health care professionals to identify appropriate support on discharge from hospital. You will have the opportunity to contribute towards providing multi-disciplinary decision making and assist individuals' make decision regarding their care needs.

In addition to professional experience of working with service users and carers you will have knowledge of the issues enshrined in The Care Act 2014, will be flexible, and have effective communication and IT skills. In return for your commitment we will provide a supportive team environment with access to professional development opportunities. You will also receive regular supervision, and be supported to attend training to progress your career.
