Are you an enthusiastic and experienced at working with people? Do you have a commitment to delivering person centred outcomes that maximise people’s independence? Are you motivated to develop your health, social care or home care assessment and support planning experience? Are you able to communicate at an excellent level with health and social care professionals, as well as the voluntary sector using a variety of methods and able to challenge other professionals reasoning in a positive and appropriate way? Do you have an understanding of the relationship between mental capacity, positive risk management and choice?

We are looking for Community Care Officers who will undertake first line prevention work, delivering short term interventions that enable independence (including Reablement), in the North, Mid and South of the county.

We offer regular professional supervision, learning and development opportunities and mentoring and support from occupational therapists and other social care staff.

We offer flexible working arrangements to enable work life balance. However, all NCC contracts state a requirement to work across a 7-day shift pattern, if necessary and when required.
