We are seeking to appoint a qualified and registered Social Worker to join the Hospital Assessment Team at Kings MIll Hospital. We are looking to recruit a committed, well organised and motivated individual who enjoys working in a busy and fast paced environment. The post is within an established and supportive team based on site at the hospital. The team works closely with the wider multidisciplinary teams within the hospital and with a range of partner agencies in the community to plan timely and effective discharges from hospital. The post will provide the opportunity to develop working relationships with a range of other professionals and an opportunity to be involved in new health and social care initiatives as they develop.

The key function of the team is to undertake Care & Support Assessments with service users and coordinate/commission services required to enable service users to be discharged from hospital. There will also be a requirement to complete a range of other assessments alongside this including: Mental Capacity Act Assessments/Best Interests Assessments, Safeguarding Assessments, and to participate in Continuing Healthcare Assessments.

The team work with adults with wide ranging needs, from those who require a period of reablement to regain their independence following a period of acute illness, to individuals with complex health and social care needs. The team focus on enabling service users to return to their own homes on discharge from hospital utilising the range of resources that are available within the community, and avoiding admission to long term care where possible.

There will be a requirement to carry a complex caseload and there is a need to be able to organise and prioritise the workload and be highly motivated in order to meet hospital discharge timescales and respond to changing priorities and demands.
