The Maximising Independence Service (MIS) is a new service in Adult Social Care which is focused on early resolution, prevention and promoting independence and wellbeing for all people.

Due to the creation of a countywide community reablement team within the service, we are seeking a creative, forward-thinking and autonomous, CQC registered Team Manager to join us who has a reablement or therapy background. The new team, made up of a number of senior reablement workers and support staff, will take a proactive role in supporting people at home to prevent the need for hospital admission and reduce the need for ongoing care – it will also free up valuable time of the existing reablement services to enable a continued focus on hospital discharge. Key to the success of the Maximising Independence Service, this brand new team is an exciting prospect and emphasises Nottingham County Council’s commitment to prevention, reablement and strength based practice.

Working closely with the rest of the maximising independence service and community social care teams, the ethos of the team is, as the name suggests, maximising a person’s independence. We will work with individuals who have experienced a period of ill health, illness or injury which has affected their ability to remain independent in their own home. We undertake regular reviews to ensure that we continue to meet the needs of the people we support and only work with people for as long as it is necessary

This role is both challenging and exciting - we would love to hear from you if you are passionate about therapy led services and your values are around promoting the principles of choice and control, personalisation and self-directed support to enable people to achieve their optimum outcomes. You will also be able to demonstrate your ability to balance busy day to day operational management with the ongoing strategic development of the team and the Maximising Independence Service. You must also have knowledge of relevant legislation and how it must be used in practice.
