We are looking for an enthusiastic and highly motivated Occupational Therapist to contribute to the work of the Short term Independence Service (STIS) in Newark.

The new STIS team is supporting people needing a safe and appropriate discharge from the non-acute A and E, CDU, and ATC departments based at Newark Hospital.

The successful candidate will be working closely with other disciplines based within the Newark teams to ensure multi-disciplinary approach to discharge planning.

The role will be based across various sites across the Newark area.

Newark STIS are involved in numerous short term rehabilitation schemes which the successful candidate would be expected to contribute to.

Knowledge of the Care Act, Mental Capacity Act and the Continuing Health Care process is essential as is experience of assessment and complex care planning with older people and their carer’s, including Section 42 work.

You must:

  • Be able to use your own initiative and communicate effectively with a wide range of people; a commitment to partnership working with Health colleagues is essential.
  • Be able to complete the full range of Assessment and Support Planning activity which incorporates, a) the outcome of a clinical based assessment; b) working to an individually tailored care plan which is person centred to enable the older person to engage in services.
  • Have good organisational, time management and IT skills.
  • Be able to adapt to changes in ways of working as a result of Service Development initiatives with Partner Agencies.

The team works flexibly in an open plan environment and offers good support and a friendly atmosphere. You will receive regular supervision and have access to appropriate training and opportunities for personal development.
