We have an exciting opportunity for a Social Worker to join the Mid Notts Discharge to Assess Team. We would like to recruit an enthusiastic, well organised, and highly motivated individual with commitment to promoting independence and a  strengths-based approach within their practice

The team work as part of an integrated multidisciplinary (MDT) transfer of care hub based at Kings Mill Hospital to plan supported discharges from Sherwood Forest Hospitals Trust (Kings Mill, Mansfield and Newark Hospitals). The role would require you to work within the integrated hub approximately two days a month alongside an advanced social work practitioner and service adviser to triage referrals and arrange short term social care support as required. This provides an excellent opportunity to work with a range of other organisations and develop your multidisciplinary working skills. The Hub MDT have a home first approach and focus on supporting people to return to their own homes on discharge from hospital where possible.

The key function of your role would be to undertake social care assessments - using the three conversations approach - with people following their discharge from the acute hospital and coordinate and commission a range of services, taking a strengths-based approach and identifying opportunities to promote individuals independence. There will be a requirement to complete a range of other assessments including Mental Capacity Act Assessments and Best Interests Assessments, Carers Assessments and to take part in Continuing Healthcare Assessments. You will also be required to undertake the safeguarding officer role and complete section 42 enquiries. An understanding of relevant Adult Social Care legislation is required, including the Care Act 2014 and Mental Capacity Act 2005. There will also be a requirement to participate in a duty rota which is completed on site at Mansfield Community Hospital.

You will receive regular formal and informal supervision, induction, relevant training and have access to mobile working technology and the flexibility of hybrid working. There will be a requirement to work some Saturdays on a rota basis which is currently 1 Saturday in 8 and there will be enhanced pay for weekend working
