We are looking to recruit a committed, well organised and motivated individual to join our team.

The team is a hospital based social care team who work with adults transferred from the acute hospital setting to Mansfield Community Hospital for a period of rehabilitation.

The key function of the team is to plan timely and effective discharges from hospital. You will be required to complete assessments of people’s social care needs and arrange the social care services that are required to enable them to be discharged from hospital. The team focus on promoting the independence of service users and utilising the range of resources that are available to support service users to return to their own home on discharge from hospital. You will work as part of a wider multidisciplinary team within the hospital and with health and social care services from outside the hospital setting.

The team also complete a range of other assessments as required, including Mental Capacity Act Assessments, Risk Assessments and participate in Continuing Healthcare Assessments as required

You will need to be able to communicate effectively with service users, the team and other professionals/agencies and have recording and report writing skills.

Any qualifications relevant to social care would be welcomed, and one years experience of working in a social care setting is required.
