We are looking for an enthusiastic and highly motivated individual to contribute to the provision of services to support older people to achieve positive outcomes reflecting individual choice. Knowledge of the Care Act and Mental Capacity Act and the Continuing Health Care process is essential as is experience of assessment and complex care planning with older people and their carer’s.

You must:

  • Be able to use your own initiative and communicate effectively with a wide range of people; a commitment to partnership working with Health colleagues is essential.
  • Be able to complete the full range of Assessment and Support Planning activity, contribute to Duty and be aware of the Safeguarding Adults Section 42 Enquiry process within the Care Act.
  • Have good organisational, time management and IT skills.
  • Be able to adapt to changes in ways of working as a result of Service Development initiatives with Partner Agencies.

The team works flexibly in an open plan environment and offers good support and a friendly atmosphere. You will receive regular supervision and have access to appropriate training and opportunities for personal development, including the ASYE if required.
