We are looking to recruit a proactive and enthusiastic person to undertake the role of Annual Review Service Organiser within the EHC Assessment Team which is part of the County Council's Integrated Children's Disability Service. Children and young people with an Education Health and Care Plan (EHCP) have a right in law to an Annual Review every twelve months (or six months when applicable). This post will provide a single point of contact for all professionals involved with EHCP Annual Reviews dealing with all queries and requests for support. You are responsible for the monitoring and tracking of all EHCP Annual Reviews. 

You will ensure that accurate records are maintained on the two current recording systems, the EHC Hub and Mosaic. You will support EHCP Coordinators and business support staff with the Annual Review process, and provide reports and the interpretation of them to ensure that that Annual Reviews are held and processed within statutory timescales. This will include the responsibility for the progress chasing of outstanding Annual Review paperwork from educational settings within the statutory timelines.

You will require a working knowledge of the Children and Families Act and Code of Practice, which sets out the legal timeframe for EHCPs and Annual Reviews and maintain a good working knowledge of service provision across the county, specifically in respect of core/universal services. You will contribute to the continuous improvement of services and their effectiveness. Safeguarding concerns identified in your work must be raised immediately with your line manager.
