The Living Well Community Team in Rushcliffe support people, and their carers, to live a good life in a place and community they call home. Our approach is based around the principles of strengths-based work to deliver the right intervention at the right time. We work closely with health and housing partners across Rushcliffe and our team includes Social Workers, Occupational Therapists and Community Care Officers.

We are looking for an enthusiastic and highly motivated person to join our team and who wants to make a difference to the lives of vulnerable adults, and their carers, living in the Rushcliffe area.

As a Community Care Officer, you will work with people to review their support needs and ensure that they are connected to services that maximise independence.

You will need to have experience of working with members of the public and the ability to work with people with complex needs. Knowledge of the Care Act 2014 and Mental Capacity Act 2005 will be an advantage in this role.

We offer a comprehensive induction and the opportunity to work in a supportive, friendly team with a 'can do' approach. You will be joining a well-established and supportive team and will access receive professional supervision and group learning sessions together with being supported to attend training to progress your career.
