The MIS North Notts Reablement Team aims to maximise the independence of people who are struggling with activities of daily living due to injury or illness. This in turn helps to prevent or reduce the need for ongoing support or residential care, by supporting people to remain safe and independent in their own homes.

Senior Reablement workers will need to have previous domiciliary care services experience and a Level 2 QCF or equivalent and be willing to work towards a Level 3. You will be trained in order to set up initial packages of care; completing assessments of the service user and the home environment to ensure that the planned service can be safely delivered. Setting initial goals in conjunction with the service user also being responsible in completing individualised risk assessments and medication administration paperwork and support plan.

Reviewing the needs of the people that are receiving the services that we deliver, working closely with the O/T staff in the team.

Seniors monitor a live scheduling system and support the staff that are on duty, training will be given, you will need to monitor and work a live system to ensure that no visits are missed, and also emergency plan for sickness/bad weather conditions.

All staff have electronic devices.

The service is registered with the Care Quality Commission. All Seniors are expected to have or work toward the NVQ Level 3 (or equivalent) in Social Care.

Good interpersonal and communication skills are essential. Staff must have use of a car for work.

Your work rota will reflect the need to provide services 7 days a week, on a four on/four off rota pattern between the hours of 7am & 10pm.
