Bassetlaw Hospital's Integrated Discharge Team ensures that hospital patients with care or rehabilitation needs have a well planned, safe and timeley discharge.
We have a full time, permanent vacancy for a qualified Social Worker to join us at this busy, friendly District Hospital.
Candidates need to have a strong ethos of multiagency, multidisciplinary working and a commitment to empowering service users to participate fully in discharge and support planning.
Candidates will need to be prepared to work over some weekends and Bank holidays as agreed
Grading from Band A to Band B according to experience.
