We have an exciting opportunity within the Adult Social Care Team as Queens Medical Centre for a Service Advisor under our apprentice scheme.

With the increasing demands to ensure that patients at Nottingham University Hospitals are not delayed unnecessarily in acute hospital settings we operate a 'no body waits' culture working with partner agencies to ensure that patients are discharged home with the appropriate support. All agencies work together to monitor, track and report on patients during their hospital stay and this role enables us to consider a social care perspective to discharge planning.

We are looking for a reliable, enthusiastic person with good attention to detail, with experience of using Excel spreadsheets and reports produced by them. The successful candidate with also be required to screen all referrals from NUH sits, monitor and refer onto reablement services were appropriate. Good communication and interpersonal skills are essential as the successful candidate with be expected to work within an integrated discharge team and liaise with colleagues from various departments and organisations on a daily basis. They must have the ability to gather appropriate information from a number of sources including health and social care records,  support with ensuring patients are on the correct care pathway, populate a spreadsheet, ensure the flow of work into the social work team and ensure that records are completed in a timely manner.

Candidates must be over 18 years.
