Advanced Social Work Practitioner
A part time Advanced Social Work Practitioner is required for this friendly, award winning Integrated Discharge Team. They will work in partnership with their Team Manager, the Discharge Lead Nurse and all members of this busy multidisciplinary team to ensure that people in hospital have a well planned, safe and timely discharge. The postholder will provide professional supervision and leadership to staff, as well as being responsible for a caseload of highly complex and specialist work. They will have a particular lead on Safeguarding, service delivery improvements and professional development.
You will need be prepared to work over some weekends and Bank Holidays as agreed.
