We have an exciting opportunity for a Social Worker to join the Mid Nottinghamshire Discharge to Assess Service. We are looking to recruit an enthusiastic, committed, well organised and highly motivated individual who enjoys working in a busy and fast paced environment. We will consider both newly qualified and experienced social workers for this role.

The Mid Nottinghamshire Discharge to Assess Service is responsible for arranging social care services for discharges from Sherwood Forest Hospitals Trust (Kings Mill, Newark & Mansfield Community Hospitals) and for undertaking social care needs assessments with people once they have returned home to identify what ongoing support they require. The team also provide social care and occupational therapy assessments and interventions to the following services: Poppyfield’s Housing With Care Assessment Apartments, Transfer to Assess Rehabilitation beds, people receiving rehabilitation at home through the Community Urgent Response and Rehabilitation Team and people receiving mental health support at home from the Intensive Home Treatment Team. There are therefore good opportunities to be involved in a wide range of work across different service areas.

You will also provide an excellent opportunity to work in partnership with both acute and community health providers and other organisations, and also to be involved in the development of integrated working arrangements within this service area as the discharge to assess model is further developed over the forthcoming months.

The team has a home first ethos and is committed to supporting people to continue living within their own homes and to identifying opportunities for reablement and promoting independence. You will be required to work in partnership with people with care and support needs and their carers to undertake social care needs assessments taking a strengths-based approach. Alongside this there will be a requirement to undertake section 42 enquiries, mental capacity assessments, best interests decisions and participate in continuing healthcare assessments.

You will also be required to take part in a duty rota and there will be some weekend working as part of the role, which is currently working one Saturday every 8 weeks to support weekend discharges from the hospital, for which there is a pay enhancement.
