Nottinghamshire County Council recognises the essential contribution which Occupational Therapists make towards preventing and reducing the ongoing support needs of people referred for personal care support. 

The discharge to assess service Mid Notts provides social care assessment and intervention to Kings Mill Hospital, Mansfield Community Hospital, Newark Hospital, transfer to assess beds, assessment flats and beds, and community health services.

Occupational Therapists are integral members of the discharge to assess service, which supports hospital discharges and reduces the risk of hospital and care home admissions.

The teams have a home first approach and focus on enabling service users to return to their own homes on discharge from hospital using the range of resources available within the community and avoid admissions to long term residential care directly from hospital.

Discharge to assess OTs utilise their specialist skills to:

  • Assess individuals’ functional abilities, needs and potential for independence.
  • Assess for and implement reablement programmes that are person centred, have clear goals and are tailored to the individual’s needs.
  • Demonstrate appropriate rehabilitative techniques to service users, carers and staff in order to maximise independence.
  • Contribute towards embedding reablement techniques across adult social care and health.

You will cover the Mid Nottinghamshire district including Mansfield and Ashfield. The base is currently at Mansfield Community Hospital.

NCC staff enjoy access to the latest mobile working technology and professionally led clinical support and supervision.

You will be required to work flexibly which may include some weekends/Bank Holidays as requested.
