We are looking for an enthusiastic and highly motivated individual to contribute to the MHSOP Intensive Recovery Intervention Service (IRIS). The aim of the service is to prevent avoidable hospital admissions for older people with mental health needs by providing an intensive mutli-disciplinary assessment and specialist care in their own homes.

Knowledge of the Care Act and Mental Capacity Act and the Continuing Health Care process is essential as is experience of assessment and complex care planning with older people and their carer’s, including Section 42 work.

You must:

  • Be able to use your own initiative and communicate effectively with a wide range of people; a commitment to partnership working with Mental Health colleagues is essential.
  • Be able to complete the full range of Assessment and Support Planning activity which incorporates: a) the outcome of a clinical cased assessment; b) working to an individually tailored care plan which is person centred to enable the older person with mental health needs to engage in services.
  • Have good organisational, time management and IT skills.
  • Be able to adapt to changes in ways of working as a result of Service Development initiatives with Partner Agencies.

The team works flexibly in an open plan environment and offers good support and a friendly atmosphere. You will receive regular supervision and have access to appropriate training and opportunities for personal development.
