We are looking to recruit a dynamic, self-motivated Occupational Therapist to join our well established Multi-Disciplinary Team based at Lawn View House, Sutton in Ashfield.

You will be working with adults aged 18-64 who live in the Ashfield/Mansfield locality who have complex physical needs and long term conditions. This can also include co-morbidities of learning disability, mental health or substance abuse.

You will have the responsibility for the assessment and provision of a wide range of services, including: equipment, moving and handling, single handed care, rehousing and major adaptation. In addition to this you will have an exciting opportunity to develop a new model of joint assessment with our Social Work colleagues to ensure the promotion of independence and reduction of dependence upon care provision through early assessment and provision of OT solutions. You will also work in collaboration with our wider team, our local housing providers, equipment providers, our health colleagues and any other relevant agencies.

You will be fully supported to maintain your CPD portfolio and will have opportunities for ongoing professional and personal development. You will need good I.T. skills, and experience of working independently and managing your own caseload. Knowledge of specialist equipment, DFG processes and moving & handling is desirable
