We have a vacancy for a Community Care Officer within the Living Well Broxtowe Community Team. Our multi-specialism team works across the Broxtowe district with adults with mental health issues, physical and learning disabilities, and autistic spectrum disorder. This role would be within the learning disability specialism but there may be opportunities to develop skills within other specialisms over time.

The team takes a strengths-based approach and we need someone who is enthusiastic and committed to enabling people to lead their lives as independently as possible. You will support people to use their own skills, resource and networks to prevent or reduce dependence on social care services.

The role will involve assessing needs and reviewing support plans, along with a wide range of case management tasks. You will take part in the team’s duty rota, across all specialisms, with support as needed. You will be joining a supportive and motivated team, with a collaborative approach to team working.

You will have experience of working with people with learning disabilities or other vulnerable groups and will be familiar with the Care Act and Mental Capacity Act and working within these frameworks. You will have good organisational and communication skills and be able to build positive relationships with people with a range of needs and abilities, and from a range of different professions and organisations.
