The Younger Adults Project Team has 2 permanent social worker posts available in the care management side of the team.

The primary base for this post will be Ollerton, but you will have a county wide brief. 

Your role will be to carry out social care assessments and commission services for people who are detained in hospital under the mental health act. You will be working with patients in both learning disability and mental health hospital settings -assessing need and developing discharge plans.You will have a small, complex caseload and will be required to represent the department at a variety of meetings, including mental health tribunals and Care and Treatment reviews. You will need to be able to work effectively alongside hospital and community multi-disciplinary teams. You will need to be able to form positive relationships with key health and social care commissioners and contribute to the development of services. Depending on demands, you may be required to play a role in the broader activity of the team. This could include project management work around the setting up of new services.

you will need to hold a driving licence and have the use of a vehicle for work purposes
