Connect Link Worker
Based: Home and field based.
Connect provides support to people at risk of losing independence because of finances, housing, physical and mental health, social isolation, care needs or any other issues concerns affecting their wellbeing.
You will provide telephone and face to face support to empower people to find solutions to their problems. With an awareness of the issues vulnerable people face you will be empathetic, sensitive and able to empower people to self-manage their needs.
Self-motivation and discipline are key as you will work from home and manage a busy caseload.
