The Maximising Independence Service (MIS) is a new service area in ASCH which is focused on early resolution, prevention and promoting independence and wellbeing for all people. Its purpose is to help people to maximise their independence as much as possible by providing information, advice and guidance, setting short-term goals and identifying support to achieve those goals.
It incorporates Occupational Therapy-led Reablement Teams, which deliver individually tailored reablement programmes designed to prevent and reduce ongoing care and support needs.
You will work both with our in-house Reablement Team and selected care agencies, which are commissioned to provide a short term homecare service to facilitate hospital discharges and meet urgent care needs.
Advanced remote working technology is provided to maximise efficiency, minimise travel and support work/life balance.
