This post will be based predominately with the Short Term Assessment Beds based at Woods Court and Bishops Court Care & Support Centres.   This  post now comes under the Short Term Independence Service (STIS) structure which brings together Social Care resources which fall within Intermediate Care which include the  Intensive Recovery Intervention Service (IRIS) and the Short Term Assessment & Re-ablement Team (START).  As part of your role you will also be required to work across the different teams in order to create a flexible and responsive service.

This is an exciting opportunity to be part of an ever changing Social Care team with the main objectives being to re-able people, maximise their potential for recovery, skills and well-being

You will be expected to: - 

Manage a complex caseload of vulnerable adults living with a range of complex conditions. This will include Mental Capacity assessments, DST’s and safeguarding work. The post involves close working with health colleagues in undertaking assessments and reviews.

Support managers to meet the departments’ performance and service delivery requirements, to manage systems effectively and to work positively within a culture of change. The ability to be resilient and effective in supporting staff and managers in the current challenging financial climate. 

Work autonomously as part of a multi-disciplinary team, good working knowledge of the assessment processes and the role of the Social Worker. 

Effective communication, organisational and IT skills are essential. You must also be able to work on your own initiative, meet deadlines whilst working under pressure, and have an awareness of, policies and procedures particularly around the Care Act 2014.
