The Mid Nottinghamshire Discharge to Assess Team has an exciting opportunity for a Qualified & Registered Social Worker to join the team, on a temporary basis to cover maternity leave until 28th February 2022. We are looking to recruit an enthusiastic, committed, well organised and highly motivated individual who enjoys working in a busy and fast paced environment. The post is within an established and supportive team based at Kings Mill Hospital, however the team are currently working remotely

The Discharge to Assess service in Mid Nottinghamshire provides social care assessment and intervention to Kings Mill Hospital, Mansfield Community Hospital and Newark Hospital, and to bedded and community rehabilitation services, assessment flats /beds, and the IRIS service within the locality. The team has a home first ethos and focus on enabling individuals with care and support needs to remain in their own homes using the range of resources available within the community.

The post provides the opportunity for multidisciplinary working and integrated working with acute and community health trusts, voluntary sector organisations and district councils, alongside partnership working with individuals, their families and carers.

You will be required to undertake care and support assessments, and coordinate and commission services required to enable people to be discharged from hospital, taking a strengths based approach to practice and identifying opportunities for reablement and promoting independence. There will also be a requirement to complete a range of other assessments alongside this including: Mental Capacity Act Assessments/Best Interests Assessments, Safeguarding Assessments.

There will be a requirement to carry a complex caseload and there is a need to be able to organise and prioritise the workload and be highly motivated in order to meet hospital discharge timescales and to respond to changing priorities and demands.

An induction and access to relevant training will be provided

You will be required to work flexibly across the service to meet demand and some weekend working on a rota basis will be required as the team provides a seven-day service.
