We are looking to recruit an enthusiastic, committed, highly organised and motivated individual to an Advanced Social Work Practitioner Post in the Adult Social Care Assessment Teams within the discharge to assess team mid Notts.  This is an exciting opportunity to work within a supportive team in busy, fast paced work environment. This post will provide lots of opportunities for professional development including developing integrated working with a range of health colleagues both within the hospital trust and community services, and the opportunity to develop your management and leadership skills alongside your skills in complex case management.

The discharge to assess service Mid Notts provides social care assessment and intervention to Kings Mill Hospital, Mansfield Community Hospital, Newark Hospital, transfer to assess beds, Assessment flats and beds, community urgent response and rehab team and the IRIS service.

The teams have a home first approach and focus on enabling service users to return to their own homes on discharge from hospital using the range of resources available within the community and avoid admissions to long term residential care directly from hospital.

The postholder will have supervisory responsibility for some of the staff within the team and will be required to hold a small caseload of complex cases alongside this. You will be required to support team managers by attending a range of meetings with health colleagues both related to daily discharge work and to the development of integrated discharge processes. You will need experience of safeguarding adults work and will be required to undertake the safeguarding manager role which will involve joint working with the hospital trusts internal safeguarding team. You will share in the responsibility for managing the high volume of work coming into the team to meet discharge timescales and to maintain Nottinghamshire County Council’s excellent position with regard to delayed transfers of care.  You will need to have the skills and ability to advise and guide others with the management of their own cases and undertake joint case work when this is required to resolve difficulties that may arise. You will also be required to support team managers in connection with the recruitment and induction of staff and provide opportunities for the continuous professional development of team members. 

You will be provided with an induction and relevant training.

There is a requirement for this post to hold the BIA or AMHP or Practice Educator (Level 2) qualification and to be working as an experienced Band B Social Worker or equivalent.
