Part Time Community Care Officer
The postholder will be an integral part of the multidisciplinary Integrated Discharge Team which ensures that patients in hospital have a well-planned, safe and timely discharge; with appropriate support packages if required.
Community Care Officers carry out initial assessment of need - Fact Finds - as Trusted Assessors within the Team to determine discharge pathways.
Where social care support is indicated they will complete more detailed Community Care Assessments; and where necessary arrange short term care placements; ongoing packages of care and carry out mental capacity, best interest and carers assessments.
Candidates must have at least two years relevant experience working in a health or community care setting; along with excellent communication and team-working abilities.
