This is an exciting opportunity for motivated, enthusiastic and organised Community Care Officers to join an energetic and dynamic Discharge to Assess team, working across Nottinghamshire South covering Rushcliffe, Broxtowe and Gedling locality areas. 

We are a team of Social Workers, Occupational Therapists and Community Care Officers who are committed to ensuring that people are discharged from hospital safely, with the right support and ensuring any ongoing rehabilitation needs are identified to enable people to live as independently as possible. We form part of the Acute/sub-acute discharge services in South Notts, supporting discharges out of Nottingham University Hospitals and Lings Bar rehabilitation Hospital, completing assessments both in the hospital and in the community.

Acute/sub-acute services consist of the Intermediate Discharge Team (IDT), which is a hospital-based service, and Discharge to Assess Team (D2A) which provide follow up support in the community. Both of these vacancies are within the D2A team, however, you will be required to work flexibly during times of need and this may include weekend working on occasions.

One of the vacancies is at Lings Bar Hospital where the focus is on timely discharge planning. The others are in the wider Discharge to Assess Team.

You will be required to commission services and monitor packages of care within set timescales. You must have knowledge of community care legislation such as The Care Act 2014, Mental Capacity Act 2005, Carers Act, etc. and relevant policies and procedures and be committed to providing a high-quality service. Ongoing training is provided and is an essential aspect of this role. You will contribute to the strategic development of the service including Strengths Based Approaches, proportionate assessments and outcome focused personal budgets. Our philosophy 'Home First', recognises that people’s needs are best met in their own homes and upholds Care Act values of promoting independence. We work closely with our colleagues in Health to promote holistic assessments of need, and jointly with people and their Cares and Families to foster a partnership way of working within a Strengths Based Approach.

The ability to communicate verbally and in writing with service users, carers and other professional colleagues is essential. Ability to work some weekends and some bank holidays is a requirement of this post.
