An exciting opportunity has arisen to work in the Ageing Well Broxtowe team for a motivated individual who wants to make a real difference to the lives of vulnerable adults. You will be joining our dynamic, hardworking and friendly team of social work staff and occupational therapists based in Beeston (covering the district of Broxtowe).

As part of the role you will undertake care and support assessments and reviews for people and their families within the Care Act 2014 and other relevant legislation. You will complete outcome focused assessments, using a strengths based approach to promote independence, choice and control to enable people to continue living in their own homes. Experience is desirable but not essential. You will work closely with a team and also autonomously and need good organisational skills. Broxtowe Ageing well team have developed excellent working relationships with health colleagues. You will be working closely with health and other keys partners in Broxtowe.
