We are looking to recruit a motivated & positive Community Care Officer to join our friendly and supportive Gedling Ageing-Well Team.

This role will involve working with Older Adults (aged 65 and over) who live in the Gedling locality who have complex physical needs and long-term conditions. This can also include co-morbidities of physical/mental health, cognitive impairments, or sensory impairments.

As a member of the team you will be expected to:

  • Assess the health & social care needs of older adults’ and work jointly and cohesively with our health partners and wider external providers in supporting older adults’ and their carers.
  • Work in a strength-based way to promote independence in achieving peoples desired outcomes and helping people stay active in their communities.
  • Have effective communication & IT Skills for which you will be provided with appropriate technology to work remotely.
  • Have an awareness of and a commitment to Equality and Diversity, Health and Safety policy/principles.
  • Work autonomously and with initiative in meeting the work demands as well as taking part in a busy duty system – this will involve working to deadlines, prioritizing work and being resilient.
  • Support managers to meet the departments’ performance and service delivery requirements, to manage systems effectively and to work positively within a culture of change.
