We are seeking to appoint a qualified and registered social worker to join the Hospital Assessment Team at Kings Mill Hospital. We are looking to recruit an enthusiastic, committed, well organised and highly motivated individual who enjoys working in a busy and fast paced environment. The posts are within an established and supportive team based on site at the hospital.

The key function of the team is to undertake Care & Support Assessments with service users and coordinate/commission services required to enable service users to be discharged from hospital. There will also be a requirement to complete a range of other assessments alongside this including: Mental Capacity Act Assessments/Best Interests Assessments, Safeguarding Assessments, and to participate in Continuing Healthcare Assessments. The team has a home first approach and focus on enabling service users to return to their own homes on discharge from hospital using the range of resources available within the community, and avoiding admissions to long term residential care directly from hospital.

There will be a requirement to carry a complex caseload and there is a need to be able to organise and prioritise the workload and be highly motivated in order to meet hospital discharge timescales, and to respond to changing priorities and demands. There will also be a requirement to participate in a duty rota which involves admissions avoidance work within short stay areas of the hospital such as the emergency department.

The post will provide the opportunity for varied casework and multidisciplinary/multiagency working within the hospital, as well as the opportunity to work with a range of partner agencies in the community. It will also provide an opportunity to be involved in new health and social care initiatives as they develop, for example, integrated working processes, IT interoperability developments between health and social care. An induction and access to relevant training will be provided.

You will be required to work some weekends (one every eight weeks) on a rota basis as the team provides a seven day service and you will receive enhanced pay for weekend working.
