Community Care Officer
An exciting new team is being established to undertake reviews of adults known to our social care services, to identify support plans that maximise the potential for improved independence. The team will focus on Younger Adults from all service user groups.
This team is part of a new approach that the council is undertaking to offer a range of bespoke reviewing activity linking into the newly established Notts Enabling Service, to explore alternatives to traditional service offers.
The team will consist of X2 Occupational therapists and a Physiotherapist (seconded from Health) and 10 Community Care Officers, who will be locality patched across the county, delivering alternative solutions to traditional approaches to support.
An ability to support the development of innovative solutions to the provision of services will be vital, alongside an ability to encourage and enable your team colleagues to work in this “new” way. You will have a particular interest in supporting people to maximise their confidence and skills to live as independently as possible.
We are looking for colleagues with strong interpersonal skills, able to communicate effectively with service users and/or their carers, internal stakeholders and external partners.
You will be well organised, with experience of coordinating activities, inputting, collating and interpreting data, proactively working under your own initiative and have a high level of accuracy and attention to detail.
Computer literacy is essential to this role as there is a requirement to input and undertake actions on Mosaic. You will also have strong communication skills, be proactive and have a commitment to providing excellent customer care.
