The Department is looking for a Community Care Officers to work as part of the hospital based adult social care team. You will be part of the Adult Care Team supporting discharges from NUH, completing assessments both in the hospital and in the community. You will undertake assessments with service users and their families. They will be required to commission services and monitor packages of care, within set timescales. They must also have knowledge of legislation, policies and procedures.

You will contribute to the strategic service development of promoting independence and a discharge to assess model of working. The candidate will be part of the hospital based Social Work Team, which is a dynamic, hard working and friendly team, they need to be enthusiastic, motivated and organised in their approach to delivering a quality service and be able to work under pressure. The ability to communicate verbally and in writing with service users, carers and other professional colleagues is essential.

Training and other personal development opportunities are encouraged. This is an excellent opportunity to join a highly motivated team. These posts carry a casual user car allowance or taxi service for disabled employees. Secondments are welcomed and will need the support of their present line manager. Ability to work some weekends and on some bank holidays is a requirement of this post.
