We are seeking to recruit to a Community Care Officer post to join the discharge to assess team at Kings Mill Hospital. We would like to recruit an enthusiastic, committed, well organised and highly motivated individual who enjoys working in a busy and fast paced environment. 
The Discharge to assess service Mid Notts provides social care assessment and intervention to Kings Mill Hospital, Mansfield Community Hospital, Newark Hospital, transfer to assess beds, assessment flats and beds, community urgent response and rehab team and the IHTT community service.


The post is within an established and supportive team based at Kings Mill Hospital. The key function of the team is to undertake Care & Support Assessments with service users and coordinate and commission the services required to enable people to be discharged from hospital. This includes identifying opportunities to promote independence for, example, through the use of reablement services. There will also be a requirement to complete other assessments as required, for example , Mental Capacity Act Assessments, Best Interests Assessments and participation in Continuing Healthcare Assessments. The teams have a home first approach and focus on enabling service users to return to their own homes on discharge from hospital using the range of resources available within the community and avoid admissions to long term residential care directly from hospital. 


You will need to have an ability to organise and prioritise your workload and be highly motivated in order to meet hospital discharge timescales, and to respond to changing priorities and demands. There may be a requirement to participate in a duty rota, and this will involve the opportunity to undertake admissions avoidance work within short stay areas of the hospital such as the emergency department. 


An understanding of relevant Adult Social Care legislation is required, including the Mental Capacity Act and you will support the work of safeguarding officers and managers during safeguarding enquiries. The preferred candidates will have at least two years’ experience of work in social care either paid or voluntary and will have a relevant health/social care qualification to NVQ level 3 or equivalent, or evidence of required skills and experience.


You will receive regular formal and informal supervision and there is an expectation to complete mandatory training to further develop good practice and encourage career progression. The ability to drive and have the use of a car for work is essential, A taxi service will be provided where disability precludes this.

There will be a requirement to work some weekends on a rota basis as the team undertake seven day working, and there will be enhanced pay for weekend working
