We require a Transformation Manager to help transform services by developing and implementing plans to align and integrate Health and Social Care services across Bassetlaw.

The post will involve working closely with partners across health services as well as other key partners to deliver outcome focussed integrated solutions, ensuring that high quality, joined-up and cost-effective services are offered to the citizens of Bassetlaw. In addition, you will get involved in countywide transformation work linked to integration with health.

We are looking for a highly motivated person to join us on our transformation journey who is skilled in programme/project delivery and ideally has experience of working within both health and social care, or across this organisational boundary. We also want someone who can demonstrate leadership and ambition.

You will be joining the ASCH Team at an exciting time as the South Yorkshire & Bassetlaw and Nottinghamshire Sustainable Transformation Plans are in the early stages of development so you will have the opportunity to shape how these plans are delivered, positively impacting on the well-being and health of the residents of Bassetlaw.
