The post will be line managed by Team Managers based at Sherwood Energy Village but work will predominantly be within the Newark and Sherwood district.

The post holder will work as part of an established district team supporting people living within the community primarily aged 65+ with physical disabilities and/or cognitive impairments. This is an exciting opportunity to be part of an ever changing Social Care team aiming to transform current services.

The ultimate aim of the service is to ensure people can live independently at home by delivering community based health and social care services thereby avoiding hospital admissions.

You will be expected to: -

  1. Manage a complex caseload of vulnerable adults living with a range of complex conditions. This will include Mental Capacity assessments, DST’s and safeguarding work.
  2. Support managers to meet the departments’ performance and service delivery requirements, to manage systems effectively and to work positively within a culture of change. The ability to be resilient and effective in supporting staff and managers in the current challenging financial climate.
  3. Work autonomously as part of a multi-disciplinary team, good working knowledge of the assessment processes and the role of the Social Worker. Also to proactively engage with the Newark Older Adults District duty rota.

Effective communication, organisational and IT skills are essential. You must also be able to work on your own initiative, meet deadlines whilst working under pressure, and have an awareness of community care legislation, policies and procedures particularly around the Care Act 2014.
