A permanent 37 hour OT post has arisen for a qualified and experienced Occupational Therapist to work across x 3 hospital sites in Nottinghamshire, starting in April 18. The post would be subject to 7 day working, if required in the future.

This new OT role has been established to enable work with acute health and social care colleagues to develop, as part of a hospital discharge project that will promote prevention, independence and self-care as part of discharge planning. Alongside this OT post, there is a 18.5 project officer and three community connector posts who will work with patients to identify opportunities in the voluntary sector that will support their needs without social care input.

This OT development role involves communicating both Occupational therapy and social care values to the wider multi-disciplinary team, including: strengths based assessment and support planning, risk management, promoting therapy and short term rehabilitation opportunities for individuals, single handled care, mental capacity and safeguarding vulnerable adults.

An aspect of this role will be to train health and social care hospital based staff to shape a focused support plan at point of discharge that has rehabilitation, reablement and enablement at the core of its delivery.

This is an opportunity for an OT with experience to develop their skills in learning and development, culture change and change management. There will be considerable involvement with the acute trust, therefore excellent communication and networking skills would be required.

There will be also be a need for OT assessment and support planning skills, along with professional involvement with the wider multi-disciplinary team, for some complex hospital discharge cases.

If the successful applicant is available to start in the post earlier than April 18, there is temporary funding for an immediate start.
