The Ashfield & Mansfield Short Term Independence Service (STIS) is seeking to recruit to a permanent, full time CCO post to cover the START team primarily but also the Short Term assessment and reablement apartments at Poppyfields, IRIS and Intermediate care.

The ethos of the STIS is home first and we work with our service users to assess their needs for support on discharge from all of our services. We offer support and advice to the service user, their carer and family to ensure that any decisions made are informed. 

We are a team who work well and pull together to manage our different service areas but work independently with our own caseload. We are in a position to move the staff around to the various teams to cope where the demands are at their greatest.

The role will involve undertaking community care assessments in order to provide information for any ongoing needs identified during our assessment process. We also undertake regular reviews to ensure that our services are meeting the needs of those service users we are working with. Due to the nature of the STIS team, you may have an opportunity to work generically by undertaking a CCO OT role where you will be involved with a service user from the start to the end of the service. Training and supervision will be available to assist you with this. This is a role currently being piloted in Ash/Mans STIS.

You will be expected to: -

  1. Work as part of a multi- disciplinary team, working alongside Social Workers, Occupational Therapists and other Community Care Officers.
  2. Have a good understanding of the role of Reablement and the benefits of this approach.
  3. Undertake Community Care Assessments in order to establish ongoing care needs and undertake reviews to ensure that there is a flow through the START service. 
  4. Work in partnership with Care Providers to ensure best outcomes for service users and their families.
  5. Have a knowledge of community care services, knowledge of relevant legislation, particularly the Care Act (2014) and the Mental Capacity Act (2005).
  6. Have an understanding of issues affecting adults in this client group and their carers is essential.
  7. Have an awareness of safeguarding adult procedures.

Effective communication, organisational and IT skills are essential. You must also be able to work on your own initiative and meet deadlines whilst working under pressure, and have an awareness of and a commitment to Equality and Diversity, Health and Safety policy/procedures.

You may also be asked to work a 7 day week as the service develops.
