Gedling Care Delivery Group (CDG) is seeking to recruit a Community Care Officer to join the team. The post is temporary until March 2018 (and potentially further). You will have experience and knowledge of adult social care and able to demonstrate that they can work in a person centred way while adopting the Adult Social Care Strategy and its emphasis on maximising independence, preventing and delaying the need for care and working in collaboration with the voluntary sector and informal carers. Core tasks will include assessment, support planning and risk enablement alongside utilising the range of resources available within the community to prevent unnecessary admission to hospital and residential care.

The CDG service supports patients in a truly multi organisational and multi-dimensional approach to reduce health deterioration, premature death and avoid hospital and residential admission. The service is part of a pilot bringing together health, social care and voluntary services, to test out the principles of working together better for the benefits of the patients. It is also the intention to support the urgent care agenda through reducing risk, risk of hospital admissions and readmissions, health deterioration and proactive care management across the sectors; as well as joining budgets to reduce duplications and identify efficiencies across the system. The CDGs consist of a wide range of health professionals from primary, community care and the voluntary sector.

There will be a requirement to carry a caseload, contribute to duty systems while organising and prioritise workload in order to address the referrals received and respond to changing priorities and demands. This will include attending regular Multi-Disciplinary Team meetings at GP Practices and maintaining a close working relationship with health and voluntary sector colleagues. The posts are located both within the GP Practices within Gedling and with the Gedling Older Adults Team and the CDG Social Worker will move between these bases.
