ICDS Assessment Team- Caseworker
The Integrated Children’s Disability Service, Assessment Team are a county wide service responsible for progressing assessments for and maintaining all Education, Health & Care Plans (EHCP’s) for Nottinghamshire children, young people. This is a statutory process and has strict timescales.
You will be Required to:
Have knowledge of the Children & Families Act 2014 and the SEN Code of Practice 2015
Co-ordinate Education, Health & Care needs assessments of Children, Young People (CYP) with Special Educational Needs (SEN) and/or Disabilities
Work to ensure that the voice of the CYP and their family is represented in all documents and promote co-production
Write EHC plans, ensuring that the CYP’s aspirations are reflected and that outcomes are measurable
Have good literacy & I.T skills, working knowledge of MOSAIC would be beneficial
Facilitate multi-agency meetings to agree the content of new or reviewed Education, Health & Care Plan
Manage the expectations of Families of disabled children, young people in relation to support and services and to understand our graduated response.
Be part of the Assessment Duty Rota and work to strict timescales
Make strong links with Education, Health & Care colleagues.
Be deployed across the county in-line with the needs of the service.
We are looking for a highly motivated resilient worker with enthusiasm to improve services for disabled children, young people who is able to organise and manage their work within set statutory timescales. You will be highly skilled in communicating and negotiation with experience in working effectively with parents/carers and disabled children/young people as well as a range of Education, Health & Social Care agencies and professionals.
