This post is within a well established supportive team based at Kings Mill Hospital. The team works closely with a wider multi-disciplinary team within the hospital and with a wide range of partner agencies on site and within the community to plan and facilitate effective discharges from the acute hospital.  
The key responsibility and function of the team is to undertake Care and Support Assessments with service users and carers in order to assess, commission and co-ordinate social care services. There will be a requirement to complete a range of other assessments alongside this including Mental Capacity Act Assessments/Best Interest Assessment, Risk Assessments and to participate in Continuing Health Care Assessments.
The team work with adults with a wide variety of social care needs, from people who require a period of reablement to regain their independence following a period of illness to people with complex health and social care needs requiring complex packages of care and support to enable their hospital discharge to take place. The team focusses on enabling service users to return to their own homes on discharge from hospital avoiding admissions into long term care placements.
We work to tight deadlines and timescales as stipulated by legislation and require a highly organised, flexible, motivated individual to join the team.
