The Housing with Care and Support Team require two enthusiastic Community Care Officer's to join our team. The Housing with Care and Support Team is a Countywide Team who aim to make a real difference to the lives of adults and carers delivering accommodation and care and support solutions based upon the NCC Housing with Support Strategy.

The post will require candidates to provide assessment, co-ordination of accommodation and care and support needs, and to offer support and guidance across all areas of housing need. The role will require direct work and commissioning of services with Supported Living providers and Housing providers in pursuit of appropriate accommodation.

You will need to have a good understanding of the Conversation Process, Person-Centred Planning and Strength Based Approaches, alongside The Care Act and Mental Capacity Act in order to support the team in these areas of work.

In this role you will be an active team member managing your own caseload and be expected to work alongside other professionals and non NCC agencies to manage various areas of accommodation-based needs.   

You will also be expected to work autonomously, creatively and collaboratively with a focus on achieving outcomes and targets set for the team.

It is desirable that you have experience of working with adults in various settings and an understanding of current supported living services.

You must have a current working knowledge of the relevant legislative framework that informs our work along with effective communication skills, flexibility, and essential IT skills.
