Community Care Officer

This is an exciting opportunity to join the Aging Well Discharge to Assess Team for Newark and Sherwood as a Community Care Officer. Our team is passionate about supporting local people to live independently, healthy life's in a place they call home.

If you are an enthusiastic and highly motivated and creative person with a passion for strength-based practice we want you to join our team. The assessment process for those people that you will be working with starts from when local authorities begin to collect information about the person and will be an integral part of the person’s journey through the care and support system as their needs change. At the same time as carrying out an assessment conversation, you need to consider anything else, other than the provision of care and support, that might assist the person in meeting the outcomes they want to achieve to be independent and to build a good life. In considering what else might help, you need to consider the person’s own strengths and capabilities, and what support might be available from their wider support network or within the community to help.

You will also work closely with partner agencies, including health and housing, to achieve the best outcomes for people and their carers.

You will be joining a well-established and supportive team and you will be able to access regular professional supervision and group learning sessions together with being supported to attend training to progress your career.

You will also take part in team meetings and huddles in supporting the team in the delivery of good practice.
