Community Care Officer - Ageing Well

We are looking for a highly motivated Community Care Officer to work in the Mansfield South Ageing Well Community Assessment Team based at Bull Farm Primary Care Centre, Mansfield (our NCC base is located at Lawn View House, 40 Station Road, Sutton-In-Ashfield, NG17 5GA).

The Team provides a service to individuals who are 65 years plus and individuals with working age dementia, many of whom have complex care needs requiring a multi-disciplinary approach to enable them to remain independent and safely supported within the community.

We require an enthusiastic individual and a competent assessor who has experience of working with members of the public; has experience of either paid employment or voluntary work in social care; has the ability to work with people with complex needs; and undertake care and support assessments and commission related care packages (initiating a full range of service requests, recognising the need to involve other more experienced staff as necessary). You will be expected to undertake Mental Capacity Act assessments and best interest decision making. You will also be expected to maintain a caseload which will include risk management work with the appropriate oversight and direction of the line manager/supervisor.

In addition to professional experience of working with service users and carers you will have knowledge of the issues enshrined in The Care Act 2014, will be flexible, and have effective communication and IT skills.
