We are looking for a Community Care Officer interested in assessing and support planning for young people who are approaching Adulthood. Working with young people and their Families you will be coordinating a strength-based plan, promoting independence and high aspirations. Preparing for adulthood is a positive and creative area of social care where we have an opportunity to develop, with the person their outcomes and plans to achieve them. You will be a great listener and want to work collaboratively with people to find the right solutions and create support plans that are built with the person at the centre. 

The PFA team works as a multi specialism team and we meet young people with a variety of assets and challenges. We focus on Preparing for adulthood outcomes, good health, friends and relationships, housing and accommodation, support in the community, education and employment. Our caseloads are varied and manageable. We hold regular meetings to share safe good quality support planning. 

The role will involve assessing needs and reviewing support plans, along with a wide range of case management tasks. You will take part in the team’s duty rota, with support as needed. You will be joining a supportive and motivated team, with a collaborative approach to team working.

You will have experience of working with vulnerable people and will be familiar with the Care Act and Mental Capacity Act and working within these frameworks. You will have good organisational and communication skills and be able to build positive relationships with people with a range of needs and abilities, and from a range of different professions and organisations.
