This is a newly established countywide role working as part of the Integrated Children Disability Service, Short Breaks and Review Team, who are responsible for assessing for and reviewing packages of support to facilitate community short breaks. This particular post will be responsible for assessing social care needs for those children and young people who are progressing through the Education Health and Care assessment pathway. If you are able you able to encourage, support and engage with children and families to promote participation in all aspects of service delivery, then this may be the opportunity for you.

You will be required to:

  • Carry a small caseload of highly complex cases carrying out direct social care assessments to inform the EHCP needs assessment pathway.
  • Attend the weekly Multi-agency EHCP panel as a social care representative.
  • Complete tier 3 social care assessments for those cases where no other social services team are involved, and where an EHC Needs assessment is secured.
  • Have in-depth knowledge of the Children’s Act as well as knowledge and awareness of the Children & Families Act 2014; the SEN Code of Practice 2015 and SEND Regulations 2014.
  • Make strong links with Education, Health & Care colleagues.
  • Support senior case workers in preparing the local authority’s statement of case for tribunals in respect of social care needs and appropriate provision.
  • Attend court as a defendant if required.
  • To liaise with and provide support and guidance to social care colleagues in social care teams about Education Health and Care Assessments and/or plans when a case is open and active to a tier 4 social care team
  • Support multi-agency meetings as required.
  • Provide quality assurance to EHC Plans ensuring that plans represent a child/young person’s social care needs adequately.
  • Manage the expectations of families of disabled children, young people in relation to support and services and to understand the graduated approach to support.
  • Work alongside the ICDS Assessment Team offering support and guidance to case workers on social needs as necessary.
  • Support ICDS assessment team case workers in quarterly educational placement review visits where a child or young person has been placed on a residential basis.

The Person:

We are looking for a highly motivated qualified social worker with the enthusiasm to improve services for disabled children, young people. Who has a broad knowledge of social work practice and legislation. Excellent assessment and analysis skills, and highly skilled in communicating and negotiation with experience in working effectively with parents/carers and disabled children/young people as well as a range of agencies and professionals. You will be committed to using the full range of your social work skills to promote and safeguard the welfare and development of disabled children and assist their carers (including young carers) by means of creative, innovative and timely care and support planning and intervention.

You will be both forward thinking and enthusiastic and assist in developing this newly established position Experience of working with disabled children is desirable but not essential.
