The Strategic Commissioning Unit (SCU) is part of Adult Social Care, Health and Public Protection. Its role is to ensure good quality and best value services are provided to promote independence, choice, health, and well-being for a range of people including older people, those with learning disabilities, mental health issues, Autism, physical disabilities and their carers. It supports the work of the Health and Wellbeing Board and is responsible for developing a diverse range of good quality, affordable social care providers within the local market.

An exciting opportunity has arisen within the Younger Adults Reviewing Project which is part of the SCU. This Community Care Officer post will work within a small team supported by an Approved Social Work Practitioner and a Commissioning Officer. The role will be to undertake statutory assessments as part of a wider service review process and to work to identify changes which can be made to care and support packages to enable the service user to become more independent and to achieve efficiencies for the department. The candidate should be aware of the use of Assistive Technology to support the promotion of independence and confident to work with provider organisations and families to agree the best approach to meet individual need. The team are working within both Care Home and Supported Living settings with much of the focus being on reducing the reliance on paid support through the use of appropriate alternatives.

The successful candidate will be able to assess risk and think of creative solutions to meet care needs.  As this is a time limited project you will be able to develop your care management skills whilst learning within a project based environment and contributing to departmental targets. You will need to be flexible and enjoy new challenges, have good communication skills and be able to work as part of a team.

In return, as part of a small team the successful candidate will receive regular support and supervision with opportunities for joint working and team support. The post will be required to work across the County and will therefore need access to their own car. This post is temporary until the 31st March 2018.
