We are looking for someone to join our established multi-disciplinary team to carry out assessments and review existing care packages, along with a range of other care management tasks. You will have experience of working with people with physical disabilities, cognitive impairments and service users diagnosed with working age dementia. 

You will need to have a knowledge of community care services, knowledge of relevant legislation, particularly the Care Act (2014) and the Mental Capacity Act (2005). An understanding of issues affecting adults in this client group and their carers is essential.You will need to have excellent interpersonal skills and be able to confidently manage difficult and challenging situations. Experience of completing good records and report writing is essential. You will have good IT, literacy and numeracy skills. A commitment to team working and participation in the team’s duty rota will be required. The team have positively embraced the alignment of integrated working across Community Health and Social Care Staff working in Bassetlaw. Community Care Officers have a key role in moving this new way of working forward.  

This post carries an authorised user car allowance or a taxi service for disabled people.
