We are looking for an enthusiastic and highly motivated Community Care Officer who wants to make a real difference to the lives of vulnerable adults living in the Mansfield area.

This is an exciting time to join the Mansfield Older Adults Community Assessment Team, as we embed the principles of integrated working with health to deliver the right intervention at the right time in the right place.  We are co - located within a local integrated care teams and have excellent relationships with our key health colleagues including Community nursing service, Specialist nurses, Occupational Therapist, Physiotherapist and General Practitioners. Due to being in the same office there is greater information sharing and members of the public receive a quality holistic assessment from all practitioners.

The Team provides a service to individuals who are 65 years plus and individuals with working age dementia, many of whom have complex care needs requiring a multi-disciplinary approach to enable them to remain independent and safely supported within the community.

We require a motivated and competent assessor who has experience of working with members of the public; has experience of either paid employment or voluntary work in social care, has the ability to work with people with complex needs and undertake care and support assessments. The post holders will be expected to undertake community care assessments, commission care packages for older adults and their carers. You will also be required to undertake Mental Capacity Act assessments and best interest decision making.

In addition to experience of working with service users and carers you will have knowledge of the issues enshrined in The Care Act 2014, will be flexible, and have effective communication and IT skills.
