Nottinghamshire County Council is developing an improve all-age Mental Health offer, alongside its existing work for people with Learning Disability and Autism which will involve strengthened multi-disciplinary working with a variety of partners. This post will sit in the Mental Health Discharge Team and the work will primarily be with those individuals on acute psychiatric wards and involve assessment and discharge destination planning. This will promote best practice, manage demand and respond to legislative change, with a focus on reablement. We want the people we serve to live good, ordinary lives in the place they call home with the people and things that they love, in communities where they look out for one another, doing what matters to them. If people need some support, we start by understanding what a good life looks like for them, and how we can we work together to achieve it.
